GENERAL LIABILITY WAIVER AND RELEASE FORM FOR VOLUNTEERS USING CRAFT MACHINES
This Agreement is made by and between:
Patrick’s Friends
(Hereinafter referred to as the “Organization”)
AND
(Hereinafter referred to as the “Volunteer”)
1. PURPOSE OF AGREEMENT
The Volunteer agrees to participate in activities related to the Organization, which include using craft machines owned by the Organization for the purpose of .
2. VOLUNTEER ACKNOWLEDGEMENT
The Volunteer acknowledges and understands that operating or interacting with craft machines may involve certain risks, including but not limited to personal injury, property damage, or other harm. The Volunteer understands that the equipment may include, but is not limited to, specific machines or equipment, e.g., sewing machines, cutting tools, etc.
The Volunteer agrees to use the craft machines only for the intended purposes and according to the instructions provided by the Organization.
3. TRAINING AND SAFETY PROCEDURES
The Volunteer acknowledges that they have received or will receive proper training on how to safely use the craft machines. The Volunteer agrees to follow all safety guidelines, instructions, and protocols set forth by the Organization and its staff to prevent accidents or injuries while using the equipment.
4. WAIVER OF LIABILITY
To the fullest extent permitted by law, the Volunteer agrees to waive, release, and hold harmless the Organization, its officers, directors, employees, agents, volunteers, and assigns from any and all claims, liabilities, injuries, damages, or losses, including attorney fees, arising out of or in connection with the Volunteer’s use of the craft machines, even if caused by the negligence or misconduct of the Organization.
This waiver includes, but is not limited to, any injuries caused by improper use, malfunction, or failure to follow the safety procedures.
5. MEDICAL TREATMENT
In the event of an emergency during participation in activities using the craft machines, the Volunteer consents to emergency medical treatment and acknowledges that they are responsible for any medical costs incurred as a result of such treatment.
6. INDEMNIFICATION
The Volunteer agrees to indemnify and hold harmless the Organization, its officers, directors, employees, agents, and volunteers against any and all claims, demands, actions, or liabilities arising out of or in connection with the Volunteer’s actions, omissions, or participation in activities involving the craft machines.
7. ACKNOWLEDGEMENT OF UNDERSTANDING
The Volunteer acknowledges that they have read and fully understand the terms of this Waiver and Release, including the risks associated with using craft machines. The Volunteer agrees to use the craft machines and participate in the activities organized by the Organization voluntarily and understands the risks involved.
