GENERAL LIABILITY WAIVER AND RELEASE FORM FOR VOLUNTEERS
This Agreement is made by and between:
Patrick’s Friends
(Hereinafter referred to as the “Organization”)
AND
(Hereinafter referred to as the “Volunteer”)
1. PURPOSE OF AGREEMENT
The Volunteer has chosen to assist with the services provided by the Organization, which may include but are not limited to [list specific tasks or activities the volunteer will be involved in]. This document sets forth the understanding between the Organization and the Volunteer regarding the assumption of risk and liability.
2. VOLUNTEER ACKNOWLEDGEMENT
The Volunteer understands and acknowledges that their participation in activities related to the Organization involves certain inherent risks, which could result in personal injury, property damage, or other harm. These activities may include [list potential activities, such as physical labor, working with machinery, or transportation].
3. WAIVER OF LIABILITY
To the fullest extent permitted by law, the Volunteer agrees to waive, release, and hold harmless the Organization, its officers, directors, employees, agents, volunteers, and assigns from any and all claims, liabilities, injuries, damages, or losses, including attorney fees, arising out of or in connection with the Volunteer’s participation in activities for the Organization, even if caused by the negligence or misconduct of the Organization.
4. MEDICAL TREATMENT
In the event of an emergency during participation in Organization activities, the Volunteer consents to emergency medical treatment and acknowledges that they are responsible for any medical costs incurred as a result of such treatment.
5. INDEMNIFICATION
The Volunteer agrees to indemnify and hold harmless the Organization, its officers, directors, employees, agents, and volunteers against any and all claims, demands, actions, or liabilities arising out of or in connection with the Volunteer’s actions, omissions, or participation in activities related to the Organization.
6. ACKNOWLEDGEMENT OF UNDERSTANDING
The Volunteer acknowledges that they have read and fully understand the terms of this Waiver and Release. The Volunteer agrees to participate in the activities organized by the Organization voluntarily, and understands the risks involved.
